Frexalimab (development code SAR-441344) is an investigational, humanized monoclonal IgG4 antibody being developed for the treatment of autoimmune diseases, with a primary focus on relapsing multiple sclerosis (MS). It represents a novel therapeutic approach by targeting a key component of the immune synapse. Frexalimab functions as a CD40L (CD154) inhibitor. By binding with high affinity to CD40L (also known as CD154) on the surface of activated T cells, it blocks the interaction between CD40L and its receptor CD40 on antigen-presenting cells (e.g., B cells, macrophages, dendritic cells). This blockade inhibits a critical co-stimulatory signal required for full T-cell activation, B-cell differentiation, and antibody class switching, thereby dampening the adaptive immune response thought to drive inflammation in diseases like MS.
