Prezalumab (also known as AMG 557 or the proposed international nonproprietary name Praelumab) is a humanized IgG2 monoclonal antibody developed for the treatment of autoimmune and inflammatory diseases. It possesses a unique dual-targeting mechanism, designed to simultaneously bind to and neutralize two key immunomodulatory ligands: Inducible T-cell Co-stimulator Ligand (ICOSL) and B-cell Activating Factor (BAFF, also known as BLyS). By inhibiting both ICOSL and BAFF, prezalumab aims to comprehensively suppress pathogenic immune activation. It blocks the ICOS-ICOSL co-stimulatory pathway crucial for T-cell and antigen-presenting cell interactions, and concurrently inhibits the BAFF-mediated survival and differentiation of autoreactive B cells. This combined action targets multiple drivers of autoimmunity.
