- Atibuclimab (IC14) is a chimeric monoclonal antibody composed of murine variable regions and a human IgG4 Fc region. It functions as an antagonist targeting the CD14 glycoprotein. By binding to both membrane-bound (mCD14) and soluble (sCD14) forms of its target, Atibuclimab inhibits the signaling pathway initiated by microbial products like Lipopolysaccharide (LPS) (endotoxin). This blockade attenuates the resultant massive release of pro-inflammatory cytokines (e.g., TNFalpha, IL-6) that drive systemic inflammation. The IgG4 isotype is chosen to minimize complement activation and antibody-dependent cellular cytotoxicity. Atibuclimab has been extensively investigated as an anti-inflammatory therapeutic for conditions involving hyperinflammation and high levels of circulating bacterial products, such as sepsis, community-acquired pneumonia, and has also been studied for Amyotrophic Lateral Sclerosis (ALS) and Acute Respiratory Distress Syndrome (ARDS).
