Description

Bococizumab was a humanized monoclonal antibody (IgG2¦¤a isotype) developed as a proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor for the treatment of hypercholesterolemia. Like other PCSK9 inhibitors, it functioned by binding to circulating PCSK9, preventing its interaction with hepatic LDL receptors (LDLR), thereby increasing LDL receptor recycling and significantly reducing low-density lipoprotein cholesterol (LDL-C) levels. Despite demonstrating potent LDL-C lowering efficacy in Phase III trials (SPIRE program), its development was discontinued in 2016 due to high rates of injection-site reactions and the emergence of anti-drug antibodies (immunogenicity) that attenuated its LDL-lowering effect over time in a substantial proportion of patients. These factors, combined with the successful development of other PCSK9 inhibitors (alirocumab and evolocumab) with more favorable profiles, led to its termination.
